Patients with cerebellar ataxia have residual strength in their limbs but still find it difficult to walk straight.
The intended placement of the foot differs from where it actually lands, and reaching for an object may result in overshooting or trembling. Speech speed and intensity become irregular, and eye movements or swallowing may also become impaired.
This is not simply a decrease in muscle strength.
The cerebellum continuously compares the body's position and movement to regulate the magnitude, speed, distance, and sequence of force. When problems arise in the nervous system connected to the cerebellum, muscles may have strength, but multiple body parts cannot be used in the correct sequence. In cerebellar ataxia, issues with gait and balance, limb coordination, speech, eye movements, and swallowing may occur together.
Cerebellar atrophy and cerebellar ataxia do not mean the same thing.
Cerebellar atrophy refers to a reduction in cerebellar volume seen on imaging, while cerebellar ataxia refers to the presence of impairments in actual movement, balance, and coordination. The degree of atrophy does not necessarily match the decline in functional ability.

We first examine the body that has been stiffened to prevent swaying
Patients with cerebellar ataxia walk with their feet spread wide to avoid falling, keep their gaze fixed on the ground, and rigidly stiffen their neck, shoulders, abdomen, and lower back.
These compensatory mechanisms may prevent immediate falls, but over time they increase pain and fatigue in the neck, lower back, and hip joints. If the trunk is stiff and cannot rotate, the arms and legs struggle to receive stable force from the center.
Therefore, simply increasing walking distance is not sufficient.
Continuously repeating incorrect movements while fatigued can increase instability and raise the risk of falls.

The treatment goal is not to make muscles unconditionally strong.
It is to release the body that is unnecessarily stiffened and enable the patient to use their remaining neural function in actual movement.
We assess the stability response of the head and trunk from the temporomandibular joint.
Patients with cerebellar ataxia often rigidly fix their head and neck to suppress swaying.
In some patients, changing the height and lateral or anteroposterior position of the temporomandibular joint alters head and neck tension, standing center, trunk swaying, and stride length.

We apply temporomandibular joint balance therapy when changes are clear and the same directional response is repeated across multiple tests.
We do not use the device based solely on a diagnosis of cerebellar atrophy; we first check whether the patient's current nervous system actually responds to changes in temporomandibular joint position.
The current stable position changes during the treatment process.
The nervous system of a patient with cerebellar ataxia continues to change according to fatigue, sleep, disease progression, and treatment response.
Therefore, temporomandibular joint balance therapy does not permanently fix a single position. It identifies the position that responds most stably in balance with the head, neck, and trunk at that point in time.
As treatment reduces instability and excessive tension, the stable position may also change. The appliance is readjusted to match these changes.
Through repeated readjustments, we confirm whether symptoms are alleviated and whether the duration of the stable state is extended.
Constitution-based acupuncture regulates the tension in disordered meridians and tissues.
In cerebellar ataxia, the force and tension on the left and right sides are inconsistent.
One side of the trunk may brace excessively while the opposite side fails to transmit force properly. Constitution-based acupuncture uses a combination of acupoints tailored to the patient's constitution to regulate the balance of the body's meridians.

Many patients continuously tense their abdomen and lower back to prevent swaying.
When the abdomen stiffens, breathing becomes shallow and trunk rotation decreases. If the trunk cannot move, coordination of the arms and legs becomes even more difficult.
