Facial Asymmetry

Rather than forcing bilateral symmetry in photos, we correct the direction of jaw movement and the balance of facial muscles.

  • Chin deviation
  • Jaw deviation upon opening
  • Masseter/temporalis asymmetry
  • Unilateral TMJ pain
  • Asymmetric neck & shoulder tension

SUMMARY

Care Guide Summary

If your chin or lip center appears tilted to one side, your lower jaw deviates when opening, or one side of your face frequently feels tense, it is necessary to examine the bilateral responses of the TMJ and masticatory muscles.
SOIAN Korean Medicine Clinic distinguishes skeletal/dental structural discrepancies from functional asymmetry where real treatment responses can be achieved.

DETAIL GUIDE

We classify the causes and treatment responses of facial asymmetry in this way

We differentiate between static and dynamic asymmetry

Everyone's face has slight natural asymmetry. What matters is not achieving perfect left-right symmetry in photos, but whether the jaw and face continually deviate to one side, causing pain and functional impairment.

  • Whether the chin and lip center appear tilted to one side
  • Whether the lower jaw deviates to one side when opening and then returns
  • Whether one cheek and corner of the mouth engage more when smiling or clenching
  • Whether the masseter or temporalis muscle on one side feels firmer and looks larger
  • Whether clicking, pain, or catching recurs in only one TMJ
  • Whether chewing on one side is comfortable while the opposite side is rarely used
  • Whether temple headaches and neck/shoulder tension are more severe on one side

In our clinic, we do not rely solely on frontal photographs. We examine dental midline and chin position, opening and closing pathways, bilateral TMJ excursion, and tenderness in masticatory and cervical muscles. Distinguishing expressive differences from habitual jaw movement discrepancies is essential for targeted treatment.

We distinguish functional asymmetry from skeletal and dental structural differences

Functional asymmetry occurs when differences in TMJ height, movement, chewing habits, and tension in the masseter, temporalis, and neck pull the facial midline to one side. When slight positional adjustments of the jaw or muscle release immediately alter chin orientation, mouth opening, and neck mobility, there is a clear therapeutic target.

Conversely, in skeletal or dental asymmetry where jawbone growth or occlusal structure differs significantly, we do not claim acupuncture alone will alter bone morphology. However, even with underlying skeletal discrepancies, we can treat the superimposed TMJ strain, masticatory hypertonicity, opening deviation, and compensatory cervical tension.

Soian Korean Medicine Clinic actively treats modifiable functional and soft tissue issues, and clearly explains limitations from the outset when structural differences are significant. Therefore, changes before and after treatment are verified through jaw movement, muscle response, pain, and chewing function rather than a single photograph alone.

TMJ balance therapy is applied while verifying facial and nervous system responses

TMJ balance therapy is not a mechanical centering of the jaw. By finely adjusting the vertical, anteroposterior, and lateral position of the mandible, we conduct functional tests to verify how masticatory and neck/shoulder tension, mouth opening, respiration, and bilateral facial responses improve.

For patients whose chin deviation decreases, mouth opening straightens, or unilateral masseter/neck tension immediately eases upon adjusting TMJ balance, we fabricate and finely adjust customized intraoral appliances. This stabilizes sensory input from the TMJ and reduces neural tension patterns pulling the face and neck unilaterally.

When pain recurs across multiple areas—improving immediately post-treatment but quickly restiffening in the jaw and neck—evaluating TMJ balance therapy becomes especially important. Intraoral appliances are not used for every facial asymmetry case, but selected for patients demonstrating clear physical responses.

We treat facial muscles and bodily compensatory tension using facial acupuncture, pharmacopuncture, and Chuna

Facial acupuncture (Jeongan-chim) meticulously addresses bilateral tension differences and indurated soft tissues in the masseter, temporalis, eyebrows, and perioral muscles. Rather than artificially shrinking or enlarging muscles, it releases hypertonic areas and activates underused muscles to restore natural balance.

If deep tenderness and tissue stiffness persist, pharmacopuncture can be applied in conjunction. When rotation of the neck, scapulae, and torso continuously pulls the jaw to one side, FSC Functional System Chuna therapy treats movement patterns connected to the jaw.

Combining jaw repositioning with facial muscle therapy often improves not only bilateral differences in the jawline and mouth corners, but also chewing comfort, TMJ pain, temple headaches, and neck tension. SOIAN Korean Medicine Clinic does not separate appearance from function, treating the connected chain of asymmetry causing real discomfort.

We evaluate progress under standardized photographic conditions and functional tests

Because facial asymmetry can appear differently depending on lighting, angle, and expression, we compare photographs taken in standardized postures and expressions whenever possible. Alongside photos, we evaluate mandibular deviation during opening, bilateral masseter tension, unilateral chewing habits, TMJ pain, and cervical range of motion.

As treatment progresses, the mouth opens straighter without forced centering, and the duration of unilateral facial and cervical straining decreases. While visual changes emerge gradually based on soft-tissue adaptation rates, improvements in mobility and pain serve as key indicators for clinical guidance.

If unilateral facial sagging and inability to close the eye occur suddenly, or if limb weakness and slurred speech accompany the onset, it is essential to first rule out facial nerve palsy or stroke rather than general facial asymmetry.
