In chronic fatigue, we first identify why energy is depleted
There is fatigue where poor appetite and sluggish digestion prevent energy generation, and fatigue where prolonged sleep fails to restore because the body cannot relax. In other cases, stress and chronic pain cause constant tension in the jaw, neck, chest, and abdomen, draining energy even at rest.
Conversely, patients depleted of physical stamina, qi, and blood after overwork, illness, surgery, or childbirth require thorough replenishment. Even with identical fatigue, releasing accumulated tension versus replenishing true deficiency requires distinct therapeutic approaches.

Digestion and sleep must recover for restorative tonics to be properly assimilated
If postprandial bloating, drowsiness, diarrhea, or constipation recur, the body cannot fully utilize energy from food. Giving heavy tonics to such patients can aggravate abdominal fullness and bodily heaviness.
We first relieve spleen-stomach burden and abdominal tension to restore digestive comfort, then replenish qi, blood, and vitality according to constitution.
If prolonged sleep fails to refresh, simply extending sleep duration is not the solution. We assess sleep latency, nighttime awakenings, sleep bruxism, snoring/sleep apnea risk, morning headaches, and daytime sleepiness. The body must achieve deep relaxation for supplemented energy to translate into genuine recovery.

Constitution herbal medicine serves as the cornerstone of chronic fatigue care. For severe tension and stagnation, it unblocks flow; for weak digestion, it restores spleen-stomach function; for true depletion after overwork or illness, it replenishes qi, blood, and body fluids.
Constitution acupuncture regulates generalized tenderness and tension in the jaw, neck, ribcage, and abdomen, evaluating pre- and post-treatment heaviness, respiration, and fatigue. When pain and localized hypertonicity continuously drain vitality, pharmacopuncture is combined.

For general low stamina, gradually increasing walking and light resistance training aids recovery. However, if light outings, exercise, or concentrated cognitive tasks trigger severe fatigue, pain, headaches, and brain fog hours later or the next day—taking days to recover—post-exertional malaise (PEM) must be specifically evaluated.
