How We Treat the Acute Phase and Neuralgia of Herpes Zoster

We evaluate nerve responses starting from the prodromal pain that precedes blisters

Herpes zoster occurs when the varicella-zoster virus reactivates in the sensory ganglia. Burning, stabbing pain, or itching typically appears first along a unilateral dermatome, followed days later by red rashes and vesicles.

Skin lesions are only the externally visible part. If burning pain occurs alongside numbness, or if allodynia makes even light friction from clothes or blankets intensely painful, it indicates that nerves are hypersensitized following inflammation and injury.

We check which dermatome is affected, whether spontaneous pain or touch-evoked allodynia dominates, differences in skin temperature and sensation, and how significantly trunk, neck, and arm mobility as well as sleep have deteriorated due to pain.

We Start Antiviral Therapy and Korean Medicine Treatment Together from the Acute Phase

In herpes zoster, it is essential to begin antiviral therapy as soon as possible after symptom onset, especially within the initial 72 hours. Korean medicine treatment is not something you wait to start only after finishing antivirals and blister crusting.

During the acute phase, we focus on rapidly reducing new vesicles, heat sensations, swollen burning pain, and nocturnal neuralgia. Constitution-based herbal medicine is tailored depending on whether fiery heat and thirst predominate, weeping fluid and heaviness prevail, or fatigue and loss of appetite are marked.

Acupuncture treats the involved dermatome, paraspinal regions, and protective bracing in the neck, back, and ribs without irritating active blisters. Pharmacopuncture is selected when deep tenderness and sharp stabbing pain are concentrated or when surrounding muscle contracture exacerbates nerve irritation.

Antiviral therapy suppresses viral replication, while Korean medicine manages acute pain, inflammatory cascades, poor sleep, digestive weakness, and physical exhaustion. Distinguishing both modalities while applying them concurrently from the start is SOIAN's foundational principle.

After the Skin Heals, We Adapt Treatment to Residual Nerve Hypersensitivity

Even after vesicles and crusts disappear, burning, tingling, itching, and numbness can persist. Skin healing does not immediately equate to the stabilization of underlying nerve responses.

During this stage, we do not simply repeat heat-clearing acute therapies. We shift the balance of acupuncture, pharmacopuncture, and constitution-based herbal medicine depending on whether the skin is cold and numb, allodynia is prominent, or spontaneous burning pain dominates even at rest.

If pain has caused trunk flexion to one side and restricted arm movement, we also treat stiffness around the ribs, spine, and shoulder girdle. When neuropathic and myofascial pain coexist, treating only the cutaneous site cannot sufficiently restore daily mobility.

Treatment Clues Remain Even in Long-standing Postherpetic Neuralgia

When pain becomes chronic, central sensitization where minor touch is perceived as intense pain and systemic compensatory bracing to protect the painful area persist together. Even if painkillers provide temporary relief, if dressing, sleeping, and moving remain difficult, the active neural response must be treated anew.

Constitutional acupuncture treats the meridians, nerve pathways, paraspinal regions, and contralateral compensatory tension linked to the painful area. Pharmacopuncture is delicately applied to areas of concentrated tenderness and nerve irritation, while constitution-based herbal medicine is formulated to restore peripheral circulation and alleviate nocturnal pain, sleep issues, poor appetite, and fatigue.

We Monitor Progress through Ease of Touch, Sleep, and Movement

Treatment progress is not measured solely by numerical pain scores. We evaluate whether you can tolerate contact with clothing and blankets, wake less frequently at night, move your arms and torso naturally, and resume outings and work.

We also assess the frequency of burning and stabbing episodes, the area of numbness or pruritus, analgesic reliance, and the duration of relief post-treatment. Reduced anxiety around skin contact and the restoration of sleep and daily activity mark essential recovery milestones.
